AMC Health recently announced our new ability to seamlessly integrate critical patient data from the Dexcom G5, G6, and G7 continuous glucose monitoring (CGM) systems into CareConsole®, AMC Health’s clinical decision support platform.
One major benefit of this capability is that it greatly increases the pool of patients eligible for AMC Health’s virtual care programs. Patients who have been prescribed CGM devices have typically been excluded from AMC Health’s diabetes program due to the difficulties of managing patients remotely without CGM data automatically flowing into CareConsole. With this integration, however, AMC Health’s programs can be effective solutions for all persons with diabetes.
This is not a small matter. Approximately 2.4 million people in the United States use GGM devices, and that number is expected to continue to grow substantially. These users are mostly those with type 1 diabetes, but many more type 2 patients are being prescribed CGM devices.
The benefits of remote patient monitoring (RPM) and virtual care should be available to these patients, enabling them to gain better condition control from the comfort of their homes. It should also be available to the clinicians managing them, as they can leverage the real-time RPM data for more timely and effective intervention.
The integration’s other major benefit is that it provides a more comprehensive and timelier picture of a patient’s condition. Data collected from CGM devices can now be analyzed in concert with all other information collected and analyzed in CareConsole® — claims and laboratory data, data from other (non-CGM) Physiometrix and medication monitoring technologies, and survey data. This integrated analysis provides a more in-depth picture of a patient’s health than when analyzing CGM data alone.
Getting this richer analysis of a patient’s health requires no additional effort from a patient or a provider. Summary data for the past 24 hours is automatically fed into CareConsole®. Listed below are the data types captured.
This CGM data combined with other CareConsole health data helps to improve the delivery of efficient, timely, and informed care through greater detail and context.
We are now working with Abbott Laboratories to enable the integration of data from their FreeStyle Libre (2, 3, and 14) CGM systems. This project is expected to be completed in early 2024. Once finalized, AMC Health customers will be able to remotely monitor the majority of CGM users in the U.S.
Enabling the Integration of Dexcom and Abbott CGM data is a testament to the scalability of AMC Health’s solution, which was designed with expandability in mind.
AMC Health believes that a virtual care solution should be able to evolve as patient and provider needs evolve — that virtual care providers should be with their customers at every step.
