Pharmacist case management guided by remote monitoring improves blood pressure on hypertensive patients | AMC Health

22% reduction in all-cause readmit rate relative to controls, translating into ROIs >6:1

A randomized clinical trial measured the effects of pharmacist case management guided by remote monitoring on BP control in patients at 6 and 12 months, with post-intervention follow up at 18 months. The telehealth intervention group received coaching from pharmacists monitoring their BP and symptoms. The control received care from a primary care physician without remote monitoring. The telehealth cohort saw a full 11/9 mmHg improvement over the control, which was sustained at 6 months post telehealth intervention.
