Cognosante Awarded VA Remote Patient Monitoring — Home Telehealth Contract

Cognosante will provide a set of secure, patient-centric tools that monitor, educate, and help clinicians triage care on a range of conditions


FALLS CHURCH, VA — Cognosante, a mission-driven technology company, announced it was awarded a contract under the Department of Veterans Affairs Remote Patient Monitoring – Home Telehealth (RPM-HT) Program. As one of four awardees on this $1 billion contract, Cognosante and teammate AMC Health will continue to provide products and services for the RPM-HT program over an eight-year period consisting of two base years and six option years.

Under this contract, Cognosante will provide a set of secure, patient-centric tools that monitor, educate, and help clinicians triage care on a range of conditions. These tools integrate into clinical workflows and the VA electronic health records (VistA and VA Millennium), improving health outcomes and expanding Veterans’ access to care in the right setting and the right time.

This year marks the twentieth anniversary of the VHA Remote Patient Monitoring-Home Telehealth program, one of the earliest, largest, and most innovative RPM programs in the country. Since its inception, the program has progressively expanded the number of Veterans served and innovative technologies used. Many peer-reviewed publications have demonstrated the positive impact of this program regarding satisfaction and clinical outcomes.

Cognosante is a trusted partner of VA, supporting critical IT modernization initiatives, including VA Cloud Operations and Migration Services (COMS), a program which migrates legacy applications to the VA enterprise cloud, and the Community Care Referrals & Authorizations (CCRA) program, which processes 575,000 referrals for care per month. These programs have received numerous industry awards, including the FedHealth IT Innovation Award and Pinnacle Award.
